Some of your key responsibilities will include:

- In collaboration with the Post Sepsis Care Program Team, manage, plan and execute all operational and ongoing activities for the Post Sepsis Models of Care Consumer and Clinician Partnership Program
- Lead aspects of the program, including, development of detailed plans for each of the 5 work packages (WP) as part of the Partnership Program, specific documentation, program meetings and other components of the program as needed.
- Oversee and manage the planning, set up and implementation, ongoing evaluation, prioritisation, reporting and timely delivery of all operational aspects of WPs according to program plans.
